A periodic test record for all periodic tests containing the applicable code requirement(s) and date(s) performed, and the name of the person or firm performing the test, shall be installed to be readily visible and adjacent to or securely attached to the controller of each unit in the form of a metal tag. The mechanic performing the test did not have any test tags.

Feeder locations for the main and lighting branch circuits shall be marked on the disconnects. The elevator had a major alteration a year ago and the disconnects appear to be new. They need to be properly marked with feeder locations. Also, the function needs to be marked "elevator main" for the main disconnect and "elevator lighting" for the lighting.
