The battery back–up lowering device was tested and did not operate as designed, the elevator failed to return to a lower floor and open its doors. Repair the device to operate as it is designed.

This elevator has an overspeed valve device installed that is not sealed and is now required to have a 5 year full load test performed. This requirement is due to the State of Missouri adopting the A17.1 2016 version of the elevator safety code. Reinspection is required.

The cab ceiling is of the open egg-crate type, which does not provide protection from the light bulbs should they shatter. The lights shall be guarded by plastic sleeve or some other means to properly protect passengers.
