This elevator is equipped with an over speed valve that in not sealed. A category 5 test shall be performed on the over speed valve on or before the next annual inspection. The valve shall be sealed and tagged after the category 5 test.

The top floor entrance has been permanently walled off and has been taken out of service. It shall remain out of service until the following can be completed: The top floor car and hall call must be removed, the top landing door shall be bolted shut, the stop ring shall be adjusted to maintain a maximum of 24" up runby at the second landing which is now the top landing, and the hoistway limit switches shall be moved to make the second landing the top landing. Note: This elevator was locked and tagged out by TKE before we arrived today, and was put into the same condition when the inspection was completed.
