The “smoke guard” barrier device, that was installed on a number of hoistway door landings, has been activated and not reset properly. The device has been temporarily wrapped up rendering it inoperable in case of an actual emergency. Repair and reset all “smoke guard” barrier devices to operate as designed.

The car door restrictor device shall be adjusted so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. Reinspection is required.

The car traveling cable, underneath the car, is cracked through the outer insulation as the car reaches the lobby. Confirm that no internal wires have been affected and repair or replace traveling cable.
