The elevator has two large holes 2’ by 4’ cut into the back of the hoistway. The hoistway shall be repaired to be enclosed at its full length per the Missouri Minimum Standard.

This elevator has a backup emergency generator power that is used in case of a power outage. This system shall be scheduled to be tested and a reinspection is required. This requirement is due to the State of Missouri adopting the A17.1 2016 version of the elevator safety code. Reinspection required.

Elevator has emergency power selector switch at main lobby landing. Facilities manager says elevator is not on emergency power. Either provide documentation that elevator is not on emergency power and remove selector switch, or test elevator for operation on emergency power. Re-inspection required
