Fire sprinklers and heat detectors have been added to the elevator hoistway and machine room. Shunt trip disconnects shall be added and electrically tied into the heat detectors to remove power from the elevator before the application of water. Reinspection and testing will be required.

The fireman's emergency service phase I and II operations was tested and the following deficiency was noted:Phase I did not initiate when the fireman's emergency service key was turned to the “ON” position. The elevator was placed out of service. No further testing was performed, a full reinspection will be required.

The elevator ADA telephone inside the elevator is not working properly. Check and repair the programming of this phone for proper operation and passenger safety.

The machine room lighting has been changed to a motion type switch to activate. While this is an energy saving measure, it also is a safety code concern. Without motion the switch deactivates the lighting, thereby leaving maintenance personnel with no lighting. Remove motion type switch to a conventional on/off switch
