Without this sheave being secured, upon striking the counterweight buffer the sheave could move causing severe damage to the elevator which has happened in the past at other locations. Properly secure the sheave to the I-Beam.

An alteration permit shall be obtained and removal of service for that landing shall be in compliant to A17.1-2016 section 8.7. Reinspection is required and action shall be taken in a timely manner or the elevator may be taken out of service.
