The cab ceiling is of the open type, which does not provide protection from the light bulbs should they shatter. The lights shall be guarded by plastic sleeve or some other means to properly protect passengers.

|
A supplemental safety hand rail has been added to the top of the elevator car that strikes the deflector sheeve at the top of the hoistway when the elevator travels up to its full travel. The handrail is now disfigured and shall be re-fabricated to not strike the deflector sheeve to the clearances in accordance to A17.1 2004a. No red tag issued as this condition has been present for an undetermined amount of time.
