This has resulted in a number of issues listed. 1- No alteration permit has been applied for. 2-The elevator car has struck the seismic bolts that have been added to the hoistway. Clearances shall be checked. 3-Elevator car door header has been damaged by striking seismic bolts in the hoistway. 4-The seismic contractor (Paric) shall finish the work in the hoistway and certify its completion before the elevator can be returned to service. Numerous nuts were observed that were not properly secured and could potential cause damage to the elevator and the occupants inside.
