The cab ceiling is of the open egg-crate type, which does not provide protection from the light bulbs should they shatter. The lights shall be guarded by plastic sleeve or some other means to properly protect passengers.

The car door restrictor bracket is missing on the 2nd floor hoistway door. The car door restrictor device shall be adjusted so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. Reinspection is required.

The code data plate found on the equipment to indicate the code and edition that the elevator shall be inspected to indicates an incorrect referenced code. The correct referenced code shall be State of Missouri Minimum Elevator Safety Code 11 CSR 40-5.065. Provide a proper plate on the front of the controller or mainline disconnect.
