The elevator ADA telephone inside the elevator is not operating properly. Unable to establish voice communication with a monitoring service representative. Check and repair the programming of this phone for proper operation and passenger safety. Reinspection is required.

The car door restrictor device shall be adjusted so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. This requires a reinspection.
