The battery back–up lowering device was tested and did not operate as designed, the elevator failed to return to a lower floor and open its doors. Repair the device to operate as it is designed.

The cab ceiling is of the open egg-crate type, which does not provide protection from the light bulbs should they shatter. The lights shall be guarded by plastic sleeve or some other means to properly protect passengers.

When testing the audible signaling device (alarm bell) with the 110V disconnect in the machine room in the “off” position, the device failed to sound, indicating that the back-up power device (battery) has failed. Repair for proper operation. Reinspection required.
