The car door restrictor device shall be adjusted so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. Reinspection is required.

There is no code data plate found on the equipment to indicate the code and edition that the elevator was installed to, as used for all inspections. Upon investigation, it was determined that the proper code should be ASME A17.1 1996 . Provide a proper plate on the front of the controller or mainline disconnect.

The cab ceiling is of the open egg-crate type, which does not provide protection from the light bulbs should they shatter. The lights shall be guarded by plastic sleeve or some other means to properly protect passengers.
