This elevator is equipped with an over speed valve that in not sealed. A category 5 test shall be performed on the over speed valve in 2021 or 2022. The valve shall be sealed and tagged after the category 5 test.

A current MO state operating certificate shall be posted in the elevator equipment room in a noncombustible frame with a clear protective vision plate over it. The certificate displayed is expired.
