When testing the audible signaling device (alarm bell) with the 110V disconnect in the machine room in the “off” position, the device failed to sound, indicating that either the device is not hooked up to a back-up power device or the back-up power device as failed. Repair for proper operation. Reinspection required.

8.6.1.6 General Maintenance Methods
|

The hoist rope deflector sheave, at the top of the hoistway, has never been secured properly on the supporting I-beam. Without this sheave being secured, upon striking the counterweight buffer the sheave could move causing severe damage to the elevator which has happened in the past at other locations. Properly secure the sheave to the I-Beam.

A written Maintenance Control Program (MCP) shall be in place to maintain the equipment in compliance with the requirements of 8.6.The MCP shall include but not limited to examinations, tests, cleaning, lubrication, and adjustments to applicable components at regular intervals.
