2. Each hoistway landing in any elevator hoistway shall be continuously provided with a properly working door or gate. The hoistway door on the bottom landing (basement) does not open the full distance of the entrance opening which causes the elevator to become stuck and possibly creating an entrapment.

4. In checking the condition of the leading edges of the entrance panels the door striking recess on the hoistway side is rusted out and cutout leaving a bad condition for someone to become cut or injured at ankle or shin level if not careful. Ref: A17.2 4.2.1(b)(3)
