Was at facility on 1/11/2022 to complete audit phone failed to operate at that time. Facility given two weeks to have phone repaired . returned 1/25/2022 phone not operating at this time. Facility given one week (7 days) to have repaired or elevator will be Red Tagged and taken out of service.
