1. All pits shall be kept dry, clean and free of equipment or material not relating to the operation of the elevator. The elevator piston seal is leaking very badly and caused oil to accumulate in the pit.

3. Where valves and cylinders use packing glands or seals, they shall be examined and maintained to prevent excessive loss of fluid. Packing leaking extremely bad. Because this is so bad the seal could blowout and make the elevator free fall.

4. Elevators with fire service features shall comply with the edition of ASME A17.1 that the elevator was constructed to meet. Phase I fire service alarm (buzzer) doesn't function.

6. All automatic passenger elevators shall be provided with an alarm bell capable of being activated from inside the car and audible outside the hoistway. If the elevator is not equipped with a bell, a two (2)-way conversation device to the elevator and a ready accessible point outside the hoistway may be acceptable. Under the circumstances of the use for this elevator where one person could be trapped with no one else on the premises a phone will be required and a cell phone can be provided to meet this requirement.
