Portions of the bottom of the car platform have rusted away and shall be repaired or replaced. Note: This is a plywood platform with a bottom liner of sheet metal. Large portions of the sheet metal are no longer there. This has a negative impact on the fire resistive integrity, and possibly the structural integrity of the platform.

This elevator is equipped with an over speed valve that in not sealed. A category 5 test shall be performed on the over speed valve in 2021 or 2022. The valve shall be sealed and tagged after the category 5 test.
