When testing the audible signaling device (alarm bell) with the 110V disconnect in the machine room in the “off” position, the device failed to sound, indicating that the back-up power device (battery) has failed. Repair for proper operation.

The slack rope switches in the pit fail to activate when the elevator safeties are set and the hoist rope becomes slack. Slack rope switches shall be adjusted to operate as designed
