A number of hoistway doors have not had the fire retaining tabs installed on them. Fire retaining tabs are required to keep the hoistway door intact in case of extreme heat conditions. Install fire retaining tabs on all hoistway doors.

3.27 - Crosshead data plate

The elevator crosshead data plate has incorrect rope data information. The data plate states the ropes are 3/8” when the actual ropes are 10mm. Correct the information on the data plate and attach to the crosshead.

The elevator hoisting ropes are showing a high degree of wear and shall be replaced. Replace ropes within 30 days of inspection. Reinspection is required.
