Sprinkler heads have been added to the elevator machine room without the required heat detectors to activate the shunt trip disconnect before the application of water. Heat detectors shall be installed within 2’ of each sprinkler head to activate the shunt trip disconnect.

The fireman's emergency service phase I and II operations was tested and the following deficiency was noted: In phase II operation, the elevator car will not override the security feature of locked out floors on the car operating panel. Security shall be overridden in fireman’s service.

The machine room lighting has been changed to a motion type switch to activate. While this is an energy saving measure, it also is a safety/code concern. Without motion, the switch deactivates the lighting, thereby leaving maintenance personnel with no lighting. Remove motion type switch to a conventional on/off switch.
