Sprinkler heads have been added to the elevator machine room without the required heat detectors to activate the shunt trip disconnect before the application of water. Heat detectors shall be installed within 2’ of each sprinkler head to activate the shunt trip disconnect.

The ventilation fan in the controller shall be repaired to provide proper operation temperature for the elevator controller with the controller doors fully closed as designed. At this time controller doors have been removed from the controller allowing exposed wiring to be accessible to damage.

The machine room lighting has been changed to a motion type switch to activate. While this is an energy saving measure, it also is a safety/code concern. Without motion, the switch deactivates the lighting, thereby leaving maintenance personnel with no lighting. Remove motion type switch to a conventional on/off switch.
