The elevator ADA telephone inside the elevator is not working properly. Check and repair the phone line of this phone for proper operation and passenger safety.

The fireman's emergency service phase I and II operations was tested and the following deficiency was noted: - In phase II operation, the car doors will not stay in the open position when activated. Reinspection is required.

The machine room lighting has been changed to a motion type switch to activate. While this is an energy saving measure, it also is a safety/code concern. Without motion, the switch deactivates the lighting, thereby leaving maintenance personnel with no lighting. Remove motion type switch to a conventional on/off switch.
