The old traveling cable was connected by a large amount of wire butts to the new control equipment. The wiring shall not be open to public spaces and wiring shall be modified to go around the cabinet (and wire butts are a recipe for problematic issues on the elvator as this demonstrates poor workmanship from past elevaor installer). 3/29/2019

Main line disconnects are required to have 36 inches of frontal clearance per electrical code. The main line disconnect shall be relocated to meet electrical code requirements. 3/29/2019

MO Minimum : Elevators (S) - Fire Service (S)1 x The fireman's emergency service fixture requirements for both the fireman's phase I key switch at the main fire access floor and the firemen's phase II fixtures inside the elevator car do not meet the 2007 A17.1 Elevator Code currently adopted by St. Louis County which would have been required for compliance with the last major alteration (if going by the 2003 renovation only, the firemen's operation worked fine with the exception the car did not close at nudging speed under phase I recall the operation when the door protection device was rendered inoperative). 2/22/2019
MO Minimum : Elevators (S) - Fire Service (S)1 x The elevator appears to be receiving a signal for primary and alternate recall to the elevator controller in the elevator machine room, however there is not a third signal for the blinking fireman's hat inside the elevator car (we did not test the fire alarm system in conjunction which the elevator's fireman's phase I recall operation as the fire alarm system is not required to be tested during annual inspections. Hard testing is only necessary if there are modifications made to the fire alarm system or elevaor controls. The existing fire alarm system shall be modified to be provided with an additional signal to the elevator control equipment to indicate that the smoke sensor in the elevator machine room or top of the elevator hoistway has been activated so the elevator control system can additionally blink the fireman's hat jewel inside the car as required by code. It is suggested the modifications be made and tested in conjunction with the elevator contractor and inspector. 2/22/2019

A17.1-1996 : Elevators 1.3 - Operating control devices 210.1a There are no hall lanterns, nor an "In-Car" hall lantern, with appropriate ADA tones to alert passengers as to the direction of travel for the elevator. Hall lanterns in the lobbies, or an "In-Car" riding lantern in the car's return jamb, shall be provided to meet accessibility requirements per the State of Missouri under a major alteration. This really should be listed under 11 CSR 40-5.070 ADA Code however, this code is not currently available on this system. 3/29/2019

Provide a proper plate on the front of the controller or mainline disconnect. 3/25/2019

It is assumed this was temporary and all roof leaks have been corrected. We did not observe any water spots around the area either. 3/29/2019
