A permanent lighting fixture shall be provided in all pits to provide an illumination of not less than five (5) foot-candles at the pit floor. The fixture switch shall be provided and accessible from the pit access door. The lighting fixture is broken off.

The internal retaining ring of the car top stop switch is missing. When you try to put the elevator back into "Run" from "Stop" the switch pulls out of the run station.
