Upon performing the required five year full load, the elevator safety governor was found to be worn to a degree that the pull-through force was overly excessive and could potentially permanently damage the elevator safeties if testing proceeded. The elevator has been removed from service. Repair or replacement of the governor is required and a reinspection shall be performed before the elevator can be returned to service.

The safety governor rope was worn into the governor sheave and shall be replaced.

Category 5 test is incomplete, need to perform the following tests to complete inspection: 125% brake test, set safeties with capacity load, car & counterweight buffer test, pull out, pull thru test, governor overspeed switch and jaws spin test need to be performed and completed. Re-inspection required.
