The testing has not been completed on the elevator at this time, the elevator was out of service for repair. The a full category 5 test shall be performed before the inspection is completed.

There are two car door restrictor brackets missing on the 2nd floor door and fascia. The car door restrictor brackets shall be reinstalled so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. Reinspection is required.

The weight compensation chain roller guide is the counterweight before the counterweight buffer is fully compressed. Readjust the weight compensation chain roller guide to allow the buffer to fully compress.
