The platform safeties shall work. This unit is equipped with broken rope type safeties which did not actuate during the test. This unit has been red tagged and taken out of service. This item must be corrected, and a re-inspection performed before it can return to operation.

The slack rope device shall activate when the ropes lose tension, causing power to be removed from the valve and pump motor. Considerable slack in the ropes did not cause the slack rope device to actuate during the test. This unit has been red tagged and taken out of service. This item must be corrected, and a re-inspection performed before it can return to operation.

This unit is equipped with batteries which can run the unit in the case of main power failure. The batteries are no longer working and shall be replaced.
