This elevator is equipped with an over speed valve that in not sealed. A category 5 test shall be performed on the over speed valve on or before the next annual inspection. The valve shall be sealed and tagged after the category 5 test.

The pressure switch shall be activated by the loss of positive pressure at the top of the cylinder. The pressure switch is not working and shall be repaired or replaced.
