The car handrail is missing along with on of the handrail standoffs. This has created a hole exceeding .5" in diameter with nothing in place to prevent a straight through passage beyond the running clearance. This hole shall be permanently covered and a replacement handrail shall be installed.

The over speed/rupture valve is not sealed. A category 5 test shall be performed on or before the next annual inspection and the over speed/rupture valve shall be sealed.
