2.2.8 - Access to underside of car. Where the distance from the pit floor to the underside of the plank channels or slings exceeds 2100mm (83in.), with the car at the lowest landing, a means shall be permanently installed or permanently stored in the pit to provide access to the equipment on the underside of the car.

A permanent of temporary means shall be installed to allow access to the jack head and all piping structures in the pit. (Pit is very deep and jack head and piping cannot be reached from pit floor)

2.11 - Disconnecting Means and Control

Where an additional power source is connected to the load side of the disconnecting means, the disconnecting means required in Section 620-51 shall be provided with an auxiliary contact that is positively opened mechanically and the opening shall not be solely dependent on springs. This contact shall cause the additional power source to be disconnected from its load when the disconnecting means is in the open position.

The auxiliary power lowering operation shall be functional. An uninterruptible power supply for the emergency lowering operation was sitting on the floor of the machine room. When an elevator is equipped with this device, it has to be operational and can't be removed without the permission of the MO AHJ. See MO Memorandum 016 for more information regarding this violation.

This elevator is equipped with an over speed valve that in not sealed. A category 5 test shall be performed on the over speed valve in 2021 or 2022. The valve shall be sealed and tagged after the category 5 test.
