A number of hoistway doors have not had the fire retaining tabs installed on them. Fire retaining tabs are required to keep the hoistway door intact in case of extreme heat conditions. Install fire retaining tabs on all hoistway doors.

2.27.3.3-The fireman's emergency service phase I and II operations was tested and the following deficiency was noted: Phase II did not initiate when the fireman's emergency service key was turned to the “ON” position.
