The belt monitoring device, that monitors the hoist belts, was not monitoring one of the belts. The hoist belt was thoroughly inspected visually and no deterioration was detected. There was no fault code showing immediate replacement. The hoist belt monitor shall be repaired or replaced to monitor all belts used for hoisting the car.. Reinspection is required.

The elevator ADA telephone inside the elevator is not working properly. The ADA location for this elevator is incorrectly stating it as “parking garage elevator #1”. Reprogram this feature to correctly identify this elevator as “parking garage elevator #2” for proper operation and passenger safety. Reinspection required.
