Hoistways containing freight elevators shall be fully enclosed. There were two access doors noted on the rear wall of the hoistway (Top of floor 2 and top of floor 3), along with an access door in the ceiling of the hoistway that accesses the roof. All access doors shall be permanently closed (mechanically fastened or welded shut) to eliminate access to the hoistway.

The elevator machine and control equipment shall be located in a separate room or separated space. Other mechanical equipment can be located in the room but must be separated by a 6-foot fence with a self-closing and self-locking gate, operable from the interior space without use of a key. Pipes conveying liquid, gas, or vapor that cross overhead of elevator equipment shall be guarded or guttered.

The pressure relief valve on the hydraulic power unit was set too high. The unit is slated for immediate replacement and the new hydraulic pump shall conform to this section. The new relief valve shall be preset to open at a pressure not greater than that necessary to maintain one hundred twenty-five percent (125%) of working pressure.
