The fireman's emergency service phase I and II operations was tested and the following deficiency was noted: A smoke sensor shall be added to the top of the hoistway as this area is deemed a machine space with the drive machine located in the hoistway. The smoke sensor shall be tied in to perform proper phase I recall.

The mainline elevator disconnect is accessible to the “public”. The National Electrical Code that this elevator was installed to explicitly states that the elevator disconnect can only be accessed by “qualified personnel”. The elevator disconnect shall be made accessible by qualified personnel and the operation of the elevator disconnect shall not be impeded by a locking mechanism on the elevator disconnect operation.
