The elevator packing gland seeped an excessive amount of oil in testing. The packing gland shall be repaired or replaced. Reinspection required in 30 days. Elevator was taken out of service by Midwest Elevator.

The battery back–up lowering device was tested and did not operate as designed, the elevator failed to return to a lower floor and open its doors. Repair the device to operate as it is designed. Reinspection required.

3.5 - Normal terminal stopping device

The normal terminal stopping device will not activate before the car travels to the stop ring. This condition is due to the hoist ropes stretching over time. The hoist ropes shall be shortened to provide the correct runby and for the top normal directional operate as required. Repair and reinspection required in 30 days. Elevator was taken out of service by Midwest Elevator.
