When performing the unintentional movement test, the elevator car top struck the top of the hoist way damaging the railing on top of the car. The railing shall be repaired, no reinspection required.

Upon performing the required unintentional movement test, the rope gripping device failed to actuate within 48” as required. The elevator has been placed out of service at this time and a red tag issued. The rope gripping device shall be repaired and a reinspection shall be performed before the elevator can be returned to service.
