The top landing of this elevator is blocked off making it impossible to access the top of the elevator, means shall be put into place to remove the blockage of the top landing. If the blockage is not removed an alteration permit to access the elevator at a different top landing shall be obtained. If either or situations are not complied the elevator will be shut down.

The final limit switch activates due to the leveling zone not being adjusted properly from non use. The leveling zone and the final limit switch shall be adjusted to not cause entrapment. The leveling zone shall be complaint to the Missouri Minimum Code section B12.
