The elevator pit floor is covered in water. The elevator pit shall be kept clean and dry. Sealing of the pit or some sort of water containment/diversion channel shall be constructed.

The sump pump hole in the pit shall be provided with a noncombustible cover that is flush with the pit floor. There is currently no cover provided.

The car door restrictor device shall be repaired so that it will properly catch and restrict opening of the car doors to a maximum of 4 inches when the car is away from the landing zone. Currently, the battery backup system for the door restrictor is not operating correctly which is resulting in a nuisance alarm sounding continuously. Reinspection is required.

The elevator alarm bell inside the elevator is not working. Check and repair the alarm bell for proper operation and passenger safety.
