The pit depth exceeds 4 feet and there is no pit ladder provided for accessing the pit. Provide a proper, code-compliant pit ladder with handgrips extending 30 inches above the access floor for safe entry into the pit. The elevator hoistway may not be wide enough to install a pit ladder; a variance may need to be applied for.
