When performing the 5 year test, the car is loaded with 125% of the rated load. This requires testing to ensure that the car will safely stop and hold the load with the car leveled with the floor. During testing the traction sheave slipped and failed the 125% test. The elevator is placed out of service and a repair/reinspection shall be performed before the elevator is returned to operation.
