There is no code data plate found on the equipment to indicate the code and edition that the elevator was installed to, as used for all inspections. Provide a proper plate on the front of the controller or mainline disconnect.

The counterweight guides shall be adjusted to provide adequate clearance when the counterweight travels to the top of the hoistway. Currently the guides are rubbing excessively on the guide rail.

The normal terminal stopping device when activated initially stops the car as required. However, after a slight pause, the elevator continues up onto the final limit. Repair and reinspection required in 30 days.

The elevator ADA telephone inside the elevator is not working properly. Two-way communication could not be established. Repair/replace this phone for proper operation and passenger safety. Reinspection required.

This elevator has a backup emergency generator power that is used in case of a power outage. This system shall be scheduled to be tested and a reinspection is required. This requirement is due to the State of Missouri adopting the A17.1 2016 version of the elevator safety code. Reinspection required.
