The elevator feeder disconnect in the machine room has malfunctioned and will not disconnect power to the shunt trip disconnect. The elevator feeder disconnect shall be repaired or replaced to the latest version of NFPA 70. Reinspection is required.

The belts on the hydraulic pump motor are loose and slip tension under higher pressure. Tighten belts for a smooth operation of pump to motor.

The ventilation cooling fan in the machine room shall be repaired to provide proper operation temperature for the elevator controller with the controller doors fully closed as designed.
