The overspeed valve is not sealed. It shall be tested with the rated load. (2nd Notice) This violation was reported last year as well. This condition shall be corrected and re-inspected within 90 days. Failure to do so may result in revocation of your MO state operating certificate.

This elevator is equipped with an over speed valve. A category 5 test shall be performed on the over speed valve in 2021 or 2022. The valve shall be sealed and tagged after the category 5 test.
