When testing the audible signaling device (alarm bell) with the 110V disconnect in the machine room in the “off” position, the device failed to sound, indicating that the back-up power device as failed. Repair for proper operation. Reinspection required.

Sprinkler heads have been added to the elevator machine room, top of the elevator hoistway, and elevator pit without the required heat detectors to activate the shunt trip disconnect before the application of water. Heat detectors shall be installed within 2’ of each sprinkler head to activate the shunt trip disconnect.
